Is your child's immunization up to date? *
(for clients subsidized by The Region of Waterloo Child Care Subsidy, please answer yes or no)
Swimming ability: Does your child require a life jacket in shallow water? *
Children under 10 are required to wear life jackets while in the wave action pool, regardless of swimming ability.
Children must be 48" to ride the waterslides.
Do you give permission for your child to ride the waterslides that drop into a pool? *
Do you give permission for your child to ride the waterslides that run out and do NOT drop into a pool? *
For possible future advertising we need consent from parent to include photographs and video of children *

If you would like your child to be in the same camp as a friend, please provide that friend's name and the name & week of the camp(s). Children must be of the same age to be in the same camp group.
